Information for first time patients
What To Expect At Evolve
History
You will be greeted by our friendly front desk staff, and asked to fill out our New Patient paperwork with your basic information, health history, and lifestyle information. This will then be reviewed by the chiropractor with you and any additional information will be added.
Examination & diagnosis
Once the paperwork is complete, you will be taken to an exam room where your first assessment will begin. This includes a comprehensive physical examination, which combined with your history, leads the chiropractor to a diagnosis for you.
Informed consent
Your chiropractor will then ask you to sign a document that outlines your consent to both the diagnostic procedures, and the “patient care plan” discussed in the Report of findings.
Report of findings
Your chiropractor will then outline with you, your “patient care plan”. A course of action created from all the information collected from our office. It will include the suggested length of care, and the regularity of treatment specific to your personal needs, getting you back on track to feeling your best.
SOAP
SOAP (Subjective, Objective, Assessment, and Plan) notes helps your chiropractor keep track of your progress during treatment. It provides a detailed reference tool to consult during future sessions to adjust treatment plans or reevaluate diagnoses.
Chiropractors look at the underlying issues, instead of just treating the surface problems.
